HMS Medical Services
“Primary care providers work within a Patient Centered Medical Home (PCMH) model that works to improve health outcomes through care coordination and optimizing health outcomes. The PCMH model at HMS combines a new level of teamwork and technology to deliver care that is more personalized, comprehensive, coordinated, and convenient, resulting in better health outcomes.”
HMS medical doctors, nurse practitioners, and physician assistants work in a team-based approach to deliver comprehensive and integrated services with the support of highly qualified nurses, medical assistants, community health workers, care coordinators, behavioral health staff, and dental staff. Patients participate in decisions involving their health care and treatment, and HMS medical services are provided to all age groups.
